It's meant to bring some poetry to an otherwise purely technical chart. The chart is calculated using Tropical/Whole Signs parameters. It displays the twelve constellations of the zodiac, from Aries to Pisces, arranged around the wheel for illustration purposes only. The placement of the constellation doesn't match their actual position in the sky. The astrological planets and the Ascendant are accurately placed within each sign. A chart tagline briefly describes the personality traits given by the Sun, the Moon, and the Ascendant. This is a great design if you want to be somewhat poetic.
